The challenge
Clinical documentation brings administrative work together with a need for clear provenance and professional oversight. The engagement focused on supporting documentation while retaining clinician review and evidence for each note.

Governance and evaluation were developed alongside the documentation model. The workflow combined clinician approval gates with note-level lineage, making the supporting evidence part of delivery from the first release.
